Motor Vehicle Record Disclosure and Authorization Agreement

By checking the box within the Bluecrew app, I acknowledge my understanding and acceptance that Bluecrew, hereinafter referred to as "the Company," will obtain a copy of my official driving record from my State of residence. This requirement is pursuant to the engagement conditions established by the Company.

I acknowledge that any information contained within the obtained abstracts will be used strictly in line with the expectations of my role and will not violate the provisions of RCW 46.52.130 or other applicable state laws.

I am also aware that Checkr, Inc., the background report provider employed by the Company, is functioning as the agent of the Company in the process of obtaining this record.

By agreeing to this document, I am granting my explicit authorization for the release of my official driving record to the Company and its duly authorized agents. This authorization will remain valid throughout any engagement between myself and the Company.

I understand that no information contained in these abstracts will be divulged, sold, assigned, or otherwise transferred to any third person or party outside of the authorized personnel of the Company.

The abstracts of my driver records obtained in this process are to be used solely for consideration by the Company as it pertains to my engagement and responsibilities therein.
